Job description

Traceyโ€™s Place II Enhanced Behavioral Support Home (EBSH) will provide a family-like community-based environment for (4) ambulatory intellectually disabled individuals. The individuals at Tracey's Place II EBSH will identify as male or female between the ages of 18-59. The individuals residing at Traceyโ€™s Place II (EBSH) will be transitioning from a highly restrictive environment such as a Community Placement Center or State Developmental Center, such as Porterville. The facility will provide 24-hour care to individuals with challenging behaviors and/or psychiatric conditions who require enhanced behavioral support in the least restrictive setting. Tracey's Place II (EBSH) will provide a supportive environment, including additional staffing and enhanced support, to address a clientโ€™s challenging behaviors. These supportive services are beyond what is typically available in a community living arrangement. The ultimate goal is to decrease challenging behaviors, increase skills training, and utilize person-centered individualized service programs.

Traceys Place is a Enchanced Behavior Support Home providing services to Adults with intellectual and developmental disabilities (ID/DD) in the Los Angeles, CA and surrounding areas. We are a leader in the community by providing services that are based on the individualโ€™s abilities and interests. We provide a fun, safe, loving, and supportive environment for our residents.

Tracey's Place II EBSH will assist our individuals in becoming independent while living in a community setting by using a team approach, active supervision, and open communication with all members of support. This is done in an effort to reduce risk and keep the individuals and communities safe. Further, our services will strive to provide a teachable environment so the individuals can increase their daily living skills, communicate their needs, and choices, lead active lifestyles, and gain a sense of independence. Traceyโ€™s Place II EBSH ultimate goals are to:

  • Maintain dignity for all individuals
  • Teach and train staff this philosophy to effectively work with our individuals
  • Assess individualโ€™ self-help deficits and provide appropriate training needs
  • Advocate for the rights of our individuals in the society

Direct Support Professional Job Description:

Traceys Place Enchaned Behavior Support home is seeking Direct Support Professionals to provide supervision and support to Adults with Intellectual and Developmental Disabilities in the Los Angeles, CA . The Direct Support Professional will work closely with the Support Team to support Adults with Intellectual and Developmental Disabilities in a supportive, caring, and productive environment.

Responsibilities:

This is a full or part time position. The staff must have:

โ— a minimum of 6 months of prior experience providing direct supervision to Adults with intellectual disabilities.

โ— Is a Registered Behavior Techinician or LPT (Licensed Psychiatric Technician

โ— Will work with a Lead Direct Staff on duty at all times when a client is under the supervision of the facility staff.

โ— Must be at least 18 years old and have a High School diploma with proficiency in English.

Further qualifications include a criminal background check (fingerprints), medical health clearance and have a current California Driver's License with First Aid, CPR, HIV/TB. Complete a minimum of 25 hours CEUโ€™S within each 12-month cycle. Staff will be required to have Direct Support Professional (DSP) certification, which consists of two 35-hour training segments to be completed within one year of hiring date. New trainees with less than 1-year experience will complete DSP 1 within the first year. By the second year of employment workers must complete DSPProvide direct support to individuals with Intellectual and Developmental Disabilities.

  • Assist individuals with: Planning activities, planning socialization, socializing with family members, toileting or grooming, feeding, comforting or soothers, physical therapy or other treatment, medication administration or reminders of medications, etc.
  • Work cooperatively with the Support Team to support individualsโ€™ goals of living day-to-day productive activities
  • Maintain accurate documentation of individual's progress.
  • Report any changes in individual's status to the Supervisor on an ongoing basis.
  • Participate in training on a continuous basis.
  • Be able to lift a minimum of 40 lbs. as needed by individual.
  • Be able to be on feet for the majority of the work day. (Standing Walking Walking)
