Job Posting Description
Tasks include:
  1. Reviewing upcoming schedules for patients with SLO syndrome and Lysosomal Storage Disorders (cross checking against our clinic lists)
  1. Conducting follow-up phone calls with families 1 week and 1 month after the visit to review what the provider listed as action items, sending follow-up portal messages
  1. Following up with the MDs/NPs if new questions arose or if families had medical questions
  1. Reviewing the patient lists for patients that are overdue for follow-up and messaging the admin team to get those patients scheduled
  1. Collecting data on number of tasks/time to completion/use of access care coordinators/etc.
  1. Other care coordination tasks such as looking for resources and updating resource guide
