Job description

Xtreme Care is hiring a Home Care Marketer focused on Traumatic Brain Injury (TBI) referrals across NYC and Westchester County. This is a field-based role for someone who knows how to build referral relationships, educate partners, and drive intake — not a desk job.

What You’ll Do

  • Build and maintain referral relationships with:
  • Hospitals & discharge planners
  • Rehab facilities & neuro providers
  • Case managers, social workers, and attorneys
  • Present Xtreme Care’s TBI home care services
  • Attend in-person meetings and community outreach events
  • Track referrals and work closely with intake to convert them

What We’re Looking For

  • Experience in home care or healthcare marketing
  • TBI, neuro rehab, Medicaid, or waiver experience strongly preferred
  • Comfortable with in-person, field-based marketing
  • Strong communication and follow-up skills
  • Familiarity with NYC or Westchester referral networks
