Job description

ESSENTIAL DUTIES AND RESPONSIBILITIESย include the following. Other duties may be assigned.ย 

Coordinates the needs of clients and family members with resources available and provides client advocacy as needed.ย 

Assistsย clients in finding and accessing community resources needed toย maintainย independent functioning.ย 

Assistsย clients, individually and in groups, with development of independent and community living skills, social skills, interests, and leisure time activities which will encourage community involvement and integration.ย 

Assistย clients in developing an individualized recovery plan that focuses on personal and interactive problems, independent living skill needs, defining goals, andย to planย action reflecting interests, abilities, and needs.ย 

Establishes rapport with clients, assessing client needs; organizes,ย plans,ย and delivers face-to-face client services; and conductsย follow upย on the outcome of services provided.ย 

May provide services through individual or group rehabilitation to address symptoms through use of curriculum-based education and skills practice toย assistย clients in developing skills necessary to perform daily living and successful integration into community life.ย ย 

Adheres to agency policy and procedures, as found in the Forms Locked drive on each computer.ย ย Also adheres toย CARF and DMHSAS standards and criteria, federal and state confidentiality rules and regulations, and any special funding source requirementsย including Medicaid and HCA regulations.ย 

Supports and promotes a service environment for individualsย in need of behavioral health and/or co-occurring services thatย isย recovery focused, welcoming, and attentive to the needs of individuals who may have experienced trauma in their lives.ย ย Attends annual core competencies trainingย inย relationshipย toย individuals with co-occurring disorders or who have experienced traumaย in their lives.ย 

Establishes andย maintainsย professional working relationships withย supervisorย and peers.ย ย Canย effectively work within a team or group setting cohesively.ย 

Monitors eligibility status of clients for Social Security, Title XIX (Medicaid), QMB, Medicare and other benefits for which the client may qualify for andย seeksย toย assistย eligible clients in the Health Home Services/enrollment.ย 

Maintains up-to-date address, phone number, contact persons, and financial informationย onย all clients.ย 

Establishes rapport with clients, assessing client needs;ย organizes,ย plans,ย andย delivers face-to-face client services; and conductsย follow upย on the outcome of services provided.ย 

Responsible forย maintainingย strictestย confidentiality guidelines for interactionsย regardingย consumer and family information.ย 

Provide clinical services for at least 80% of caseload..ย 

Utilizes the philosophy of Collaborative/Therapeutic Documentation to meet clinical record keeping documentation standards and ensure documentation is completed with the Consumer to the extent possible and all documentation is completed on the same day of the date of service.ย ย Reports andย documentsย servicesย in accordance withย agency policy and procedures.ย 

Responsible forย seeking outย supervisoryย assistanceย forย additionalย training and to enhance skill set as needed.ย 

Interactsย with other professionals to discuss therapy or treatment,ย new resourcesย or techniques, and to share information.ย 

May beย requiredย to work evening or weekend hours.ย 

May beย requiredย to provide services in a community setting.ย 

Mustย beย able toย provide client transportation using personal vehicle or agency owned van or small bus.ย

EDUCATION and/or EXPERIENCEย ย 

  • Must meet ODMHSAS and HOPEโ€™s requirementsfor Behavioral Health Case Manager II certification.ย 
  • Case management experience in behavioral health or social services preferred.ย 

CERTIFICATES, LICENSES, REGISTRATIONSย ย 

Required to obtain and maintain DMHSAS CM Certification.ย 

Required to complete, Therapeutic Options or approved safetyย training and annual updates.ย 

May beย requiredย to obtain andย maintainย First Aid and CPR Certifications.ย 

May beย requiredย to complete andย maintainย Wellness Coach Certificationย 

In order toย provide client transportation,ย mustย possessย current, valid Oklahomaย license, meetย HOPE insurance requirements.ย ย HOPE will annually obtainย copyย of Motor Vehicle Record from the Department of Public Safety.ย
