Caregiver responsibilities:

 

  • Daily personal care such as bathing, grocery shopping, oral care, cooking meals, etc.

  • Assist and provide help with the daily living activities per the Plan of Care

  • Track your patient’s condition and/or report to the agency if there is something unusual

  • Assist patient with daily meals 

  • Basic housekeeping functions

  • Accompany patient to doctor’s appointments

  • Assist with essential errands

  • Communicate with Summit regarding any issue that your patient might have

  • Follow agency’s policies


Caregiver requirements:

 

  • New York Home Health Aide Certificate or Personal Care Assistant Certificate

  • Government issued ID

  • Social Security Card

  • Work permit, if needed

  • 2 Reference letters

  • Recent physical

  • Quantiferon 

  • Rubella and Rubeola lab work

  • Chain of Custody Drug Screen, no older than 6 months

 

We would rather have your Flu and/or Covid vaccine records but it’s no longer a requirement.
