• Conduct thorough assessments of clients to identify their unique needs, strengths, and areas for growth, utilizing standardized assessment tools and methodologies.
• Develop, manage, and implement personalized support plans that include measurable goals tailored to each client’s circumstances.
• Provide crisis intervention services and emotional support, ensuring clients feel heard and understood in their moments of distress.
• Monitor and evaluate clients’ progress on a regular basis, modifying treatment plans as necessary to align with their evolving needs.
• Maintain meticulous documentation regarding client interactions, treatment progress, and other relevant case notes.
• Participate actively in interdisciplinary case conferences to strategize coordinated care efforts and provide evidence in legal proceedings when applicable.
• Serve as a vital link between clients and community resources, effectively referring individuals to programs that further support their recovery journey.
• Engage in continuous professional development to stay abreast of emerging trends and best practices in social work and mental health care.

This position will primarily cater to a diverse population of clients dealing with multiple social challenges, including those recovering from substance abuse and the associated mental health complications. The LCSW will work closely with individuals at various stages of their recovery, empowering them through tailored interventions and compassionate guidance.

• Master's degree in Social Work (MSW) from an accredited institution is mandatory.
• Current state licensure as a Licensed Clinical Social Worker (LCSW) is required.
• Demonstrated clinical experience in providing social work services, including counseling and case management.
• Strong interpersonal and communication skills, with an ability to effectively engage and empathize with clients.

• Familiarity with crisis intervention strategies and techniques for vulnerable populations.
• Proficiency in utilizing electronic health record (EHR) systems to maintain documentation and track client progress.
• Experience collaborating within multidisciplinary teams to promote holistic care particularly concerning mental health and substance abuse.
