Job Summary

Mohawk Valley Health System (MVHS) is seeking a highly skilled and dedicated Advanced Practice Provider to join our cardiac team as a Structural Heart Coordinator. This role is responsible for coordinating and managing the care of patients undergoing advanced structural heart procedures, including pre-procedure evaluation, inpatient and outpatient management, patient education, and post-procedure follow-up. The Structural Heart Coordinator will work collaboratively with the multidisciplinary Heart Team to implement evidence-based care pathways, optimize patient outcomes, and support the continued growth and success of the Structural Heart Program.

Education/Experience Requirements

REQUIRED:    

  • Master's degree from an accredited Nurse Practitioner's Program OR certification from an accredited NP program OR completion of an accredited Physician Assistant Program required.
  • Minimum 1 year of experience in a clinical setting.
  • Ability to meet and maintain credentialing requirements and hospital priviledges.
  • Knowledge and competency in a wide range of clinical areas. 
  • Familiar with EHR/EMR software.   
Licensure/Certification Requirements

REQUIRED:    

  • Current NYS Nurse Practitioner licenses OR Current NYS license as a Physician Assistant with prescription privileges.
  • Current BCLS, ACLS certifications.

PREFERRED:

  • Active ANCC certification in relevant specialty
