We are seeking a compassionate and dedicated Mental Health Provider to join our Collaborative Care team at Carease Health - OH in North Ridgeville, OH. As a Social Worker/Care Companion, you will provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities.

About this Role

In this part-time position, you will have the opportunity to make a direct impact on the lives of older adults by helping them feel heard, supported, and emotionally connected within their environment. This role is ideal for professionals who want to work in a supportive team environment and provide meaningful resident-focused care.

Key Responsibilities

  • Visit designated facilities in accordance with the schedule set with your supervisor

  • Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs

  • Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns

  • Document visit notes and maintain detailed records of interactions and interventions after each visit

  • Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP

Qualifications

  • Associates, Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or another mental health–related field

  • Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings

  • Strong communication, documentation, and relationship-building skills

  • Education: Associate Degree

  • Experience: 1 year
