The Opportunity

We are currently seeking a dedicated Social Services Director

What You Will Do

  • Assist in the planning, organization, implementation, and evaluation of the facility’s Social Services Department

  • Ensure social services are delivered in compliance with federal, state, and local guidelines and in alignment with Harborview's values and policies

  • Help develop and monitor individualized care plans for residents, addressing their social and emotional needs

  • Provide ongoing support to residents and families during admissions, transitions, and care conferences

  • Recommend appropriate equipment, services, or referrals for outpatient and home health support

  • Participate in policy development and recommend improvements to strengthen care delivery

  • Organize and participate in in-service training sessions for the social services team and other staff

  • Stay up-to-date on regulatory changes and professional standards related to long-term care social work

  • Advocate for residents and serve as a liaison among staff, families, and external agencies

  • Collaborate with interdisciplinary care teams to ensure holistic, person-centered care

  • Duties as assigned

What You Must Have

  • Master's degree in Social Work (BSW), or in a human services field such as sociology, psychology, special education, or rehabilitation counseling. Must be willing to sit for licensure within the first 90 days of employment.

  • Minimum of 1 year supervised social work experience in a skilled healthcare setting working directly with individuals

  • Knowledge of federal and state long-term care regulations

  • Strong interpersonal, communication, and problem-solving skills

  • A resident-first mindset with a deep commitment to dignity, empathy, and respect

  • Ability to manage sensitive situations with professionalism and compassion

  • Must meet all applicable licensure and certification requirements in the state of practice
