• Conduct thorough psychosocial assessments of patients to understand their emotional, social, and environmental needs.
• Create and implement individualized care plans, employing evidence-based solution-focused therapies that foster resilience and coping mechanisms.
• Provide direct counseling services to patients and families, helping them navigate complex emotions associated with illness and loss.
• Maintain detailed, accurate, and confidential records in compliance with healthcare regulations and best practices.
• Collaborate with a multidisciplinary team, including physicians, nurses, and spiritual care providers, to develop cohesive strategies for patient care.
• Engage in continuous professional development, staying informed about emerging practices in social work and hospice care.

The primary focus of your work will be with terminally ill patients and their families, addressing various emotional and psychological responses to grief, anxiety, and depression. You will provide an empathetic ear, support structures, and resource connections to help them cope with their unique circumstances. This role requires not only clinical expertise but also deep compassion, understanding, and a commitment to enhancing the quality of life during this critical time.

• A Bachelor’s Degree in Social Work (BSW) from an accredited institution is required; advanced degrees are preferred.
• An active LCSW license in the state of Georgia.
• Strong verbal and written communication skills, with the ability to articulate complex concepts simply and compassionately.
• Experience in a hospice, palliative, or medical social work setting is highly advantageous.

Ideal candidates will have a strong background in trauma-informed care, with experience addressing psychological concerns prevalent in hospice settings, such as anticipatory grief and family dynamics during the terminal phase. Familiarity with interdisciplinary collaborative care models will significantly enhance your effectiveness in this role.
