Social Services Director

Job Summary: 

NEW FACILITY OPENING 9/1/2026. The Social Services Director is responsible for the overall operation of the Social Services department to ensure all medically related emotional and social needs of patients are met in accordance with all applicable laws and regulations. 

Essential Duties: 

  • Manage and direct the Social Services Department, and identify and provide for residents social, emotional, and psychological needs.

  • Accurately assess, document and provide interventions to meet psychosocial needs of all residents.

  • Develop care plans based on assessments.

  • Maintain a quality working relationship with the medical profession and other health related facilities and organizations.

Preferred Qualifications: 

  • Two years of job-related experience, preferably in long-term care.
