Social Services Director

Job Summary:

As a Social Services Director at Newark Rehabilitation, you will play a pivotal role in shaping the lives of our residents by providing exceptional care and support. About the Role:

As a Social Services Director, you will be responsible for overseeing the social services department, ensuring that our residents receive the highest level of care and support possible. Social Services Director will lead a team of professionals in providing comprehensive assessments, psychosocial interventions, and discharge planning to promote the well-being and independence of our residents. You will work closely with our multidisciplinary team to identify and address the unique needs of each resident, and develop and implement individualized care plans to ensure that they receive the best possible care.

Responsibilities:

• Oversee the social services department, including hiring, training, and supervising a team of social workers and other support staff
• Conduct comprehensive assessments and develop individualized care plans to promote the well-being and independence of our residents
• Provide psychosocial interventions, counseling, and support to residents and their families
• Collaborate with our multidisciplinary team to identify and address the unique needs of each resident
• Develop and implement discharge planning initiatives to ensure a seamless transition for residents leaving our facility
• Maintain accurate and thorough records of resident assessments, care plans, and interventions
• Participate in quality improvement initiatives to enhance the overall quality of care and services provided
• Develop and manage departmental budgets and ensure efficient use of resources

Requirements:

• A Bachelor's degree in Social Work, Counseling, or a related field
• A minimum of one years of experience in a social services or healthcare setting
