Job description

A well-established skilled nursing and rehabilitation center in the area is seeking an experienced and compassionate Social Services Director to join our team.

Key responsibilities include:

  • Developing and documenting individualized social service care plans.

  • Coordinating discharge planning and community resource referrals.

  • Educating residents, families, and staff regarding resident rights and available services.

  • Assisting residents and families with applications and referrals for community programs and benefits, including Medicaid and conservatorship services when appropriate.

  • Maintaining records of resident and family concerns, facilitating resolutions, and ensuring appropriate follow-up.

  • Serving as an accessible advocate and resource for residents, families, and staff.

  • Collaborating with interdisciplinary team members to promote quality outcomes and resident satisfaction.


Qualifications:

  • Bachelor's Degree in Social Work (BSW) required; Master's Degree preferred.

  • Degree from an accredited Social Work or related human services program.

  • Previous experience in long-term care preferred.

  • Knowledge of federal and state long-term care regulations, including Requirements of Participation (RoP), resident rights, admissions, discharge planning, and documentation requirements.
