Social Services Designee

The Avenue at Aurora is a premier post-hospital rehabilitation and skilled nursing facility offering a welcoming, hotel-like environment with all-private suites. We are committed to providing compassionate, person-centered care that promotes dignity, healing, and quality of life.

What You'll Do

  • Support residents with their emotional and social adjustment to long-term care and rehabilitation.

  • Advocate for resident rights, dignity, preferences, and quality of life.

  • Complete psychosocial assessments and care plans.

  • Participate in care conferences and interdisciplinary team meetings.

  • Assist residents and families with concerns, transitions, and discharge planning.

  • Coordinate community and social service referrals.

  • Maintain accurate and timely documentation.

  • Assist with admissions and provide support to residents and families.

  • Participate in quality improvement initiatives and staff education.

  • Meets Ohio requirements for Social Services Designee.

  • Long-term care/SNF experience preferred.
