As a Social Services Designee you will assist in preparing our residents for their future. Your work will ensure our residents receive the high standard of care they have grown to expect by ensuring our residents social and emotional needs are met during their stay, coordinating resident discharges and participating in resident care conferences. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

As a minimum, a high school diploma or equivalent and two (2) years of experience in a Social Worker role preferably in a healthcare setting. Prior Social Services Designee experience in a Skilled Nursing Facility, bachelor’s degree in Social work or similar field and five (5) years of related experience is preferable but we are willing to invest in the right candidate!
