Job description

Citizens Memorial Hospital (CMH) is hiring a Social Services Designee to join our Social Services Department in Stockton, Missouri - someone who leads with skill, compassion, and a drive to positively impact every patient and family we serve.
Position Details
  • Status: Part Time
  • Work Shift: Days
  • Hours: 8:00a.m to 4:00p.m, 2-3 days/week

What Your Day Looks Like: Lake Stockton Healthcare Facility (LSHCF) is a 90-bed long-term skilled nursing facility. A 30-bed memory care unit meets the specific needs of individuals with Alzheimer's disease and other forms of dementia.
As a Social Services Designee you will be:
  • Assisting with admission/discharge planning
  • Coordinating the transportation of residents to appointments
  • Delivering resident mail the same day as received
  • Assessing each resident's psychosocial status
  • Serving as a liaison between the families of residents and the health care team in time of crises
  • Making arrangements for obtaining needed adaptive equipment, clothing, and personal items
  • Assisting with family/resident concerns and/or needs
  • Utilizing the communities' resources to assist in the discharge of short stay residents

Qualifications:
Education: High school diploma, MHCA Designee Training required, will assist in obtaining
Experience: 1 year of long-term care/healthcare preferred
Skills: Collaborative, detail-orientated, problem-solving
Licensure/Certification: Family and Friends CPR within 6 months of employment
Physical Requirements: Ability to stand for 10% of the day and sit for the remaining 90%
