Pride Health is seeking an experienced Registered Nurse (RN) โ€“ Home Health to join a leading healthcare organization. This is an excellent opportunity for an RN with Home Health and Homecare Homebase (HCHB) experience to provide high-quality patient care in the home setting.


Job Details
  • Position: Registered Nurse (RN) โ€“ Home Health
  • Schedule: Mondayโ€“Friday, 8:00 AM โ€“ 4:30 PM
  • Weekly Hours: 40 hours guaranteed
  • Coverage Areas: Marion, Hamilton & Johnson Counties, IN
  • Productivity Expectation: 25โ€“30 points per week
  • On-Call/Weekend: None
  • Mileage Reimbursement: Yes (between patient visits)
Key Responsibilities
  • Complete Start of Care (SOC) assessments using OASIS documentation.
  • Perform Discharge and Recertification visits.
  • Administer medications and provide medication education.
  • Deliver skilled nursing care, including wound assessment and wound care.
  • Develop, implement, and update individualized patient care plans.
  • Accurately document all patient encounters in Homecare Homebase (HCHB).
  • Collaborate with physicians, therapists, caregivers, and interdisciplinary healthcare teams.
  • Educate patients and families on disease management, medications, and home care.
  • Ensure compliance with agency policies, Medicare guidelines, and regulatory standards.
Required Qualifications
  • Active Registered Nurse (RN) license.
  • Recent Home Health nursing experience.
  • Proficiency with Homecare Homebase (HCHB).
  • Experience completing OASIS Start of Care (SOC), Recertification, and Discharge documentation.
  • Strong clinical assessment and documentation skills.
