What You'll Do
As a Home Health RN, you'll oversee a caseload of patients within your assigned territory while partnering with physicians, therapists, and interdisciplinary team members to deliver exceptional care.
Responsibilities include:
  • Complete comprehensive Start of Care, Resumption of Care, and Recertification assessments.
  • Develop individualized care plans that support patient goals and independence.
  • Coordinate care with physicians, therapists, social workers, and caregivers.
  • Educate patients and families on disease management, medications, and safety.
  • Complete timely documentation and OASIS assessments.
  • Monitor patient progress and adjust care plans as clinical needs change.
  • Supervise Home Health Aides and collaborate with LPNs as appropriate.
  • Participate in case conferences and interdisciplinary care planning.
  • Help improve patient outcomes while maintaining compliance with Medicare Conditions of Participation and agency quality standards.
