Job Summary
Our growing home health agency is looking for part-time, per diem RN's to case manage patients in the Western Suburbs and Chicagoland area (North and South sides). Including: the counties of Cook, DuPage, Kendall, Will, Grundy McHenry, Winnebago, Kane and Lake. Previous home health experience preferred. Candidate must be willing to travel and have previous EMR charting experience. This is a part-time, fee for visit position Monday thru Friday with schedule flexibility. Preferred hours of work are 8:30am-4:30pm. Please include resume for consideration.
RN – Per Diem Pay Rate

$90 For a Start of Care (SOC)
$70 For Recert, Resumption, or Discharge
$70 For a wound patient
$60 For a Regular visit
RN – Per Diem Pay Rate – w/ Dialysis Patient and Completion of our Dialysis Education

Responsibilities 
  • Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans 
  • Administer medications and monitor patients for side effects or reactions  
  • Document any changes in patient conditions and report on changes 
  • Maintain accurate records of patient care and discuss observations with supervisor 
 
Qualifications
  • Currently licensed Registered Nurse (RN) in state of employment 
  • Previous experience as a Registered Nurse is preferred
  • Excellent interpersonal and client care skills
  • Valid driver’s license 
  • First aid/CPR certification required
