Position Summary
We’re seeking five full-time Registered Nurses (RNs) to join our Dublin, Ohio team!
As a Home Health RN, you’ll provide one-on-one skilled nursing care in patients’ homes, coordinate with physicians and care teams, and ensure top-quality outcomes for every patient.
Key Responsibilities
Conduct initial and ongoing assessments — including Start of Care (SOC), Recertifications, Resumptions, and Discharges
Develop, implement, and evaluate individualized plans of care
Administer medications, wound care, and treatments
Educate patients and families on care routines, medications, and disease management
Collaborate closely with physicians, therapists, and the interdisciplinary team
Document care accurately in WellSky EMR (or similar)
Ensure compliance with Medicare, Medicaid, and agency standards
Participate in weekend rotation as needed
Qualifications
Active RN license in Ohio
BSN preferred (not required)
2+ years of home health nursing experience
Skilled in managing diverse home health diagnoses
Strong clinical assessment and critical thinking skills
Valid driver’s license and reliable transportation
