Job Summary

Join our growing Home Health team as a Start of Care Registered Nurse (SOC RN). In this role, you will complete comprehensive patient admissions, perform OASIS assessments, develop individualized care plans, and coordinate home health services to ensure quality patient outcomes and regulatory compliance.

Key Responsibilities
  • Conduct Start of Care (SOC) and admission assessments in patients' homes.

  • Complete accurate OASIS documentation and clinical records.

  • Develop and implement individualized care plans.

  • Educate patients and caregivers on disease management, medications, and home safety.

  • Coordinate care with physicians, therapists, and the interdisciplinary team.

Qualifications

  • Active California RN license

  • Minimum 1 year of direct patient care experience (home health preferred)

  • Current CPR certification

  • Valid driver's license and insured, reliable vehicle
