Job Overview
Aarris Healthcare is seeking experienced and compassionate Per Diem Field Registered Nurses (RNs) for home health visits throughout the greater Sacramento area including Sacramento, Elk Grove, Roseville, Rocklin, Folsom, Citrus Heights, Carmichael, Rancho Cordova, Davis, Woodland, and surrounding communities.
We are looking for nurses who thrive in an independent field environment and are passionate about providing high-quality skilled nursing care to patients in their homes.
Flexible schedules available.
Responsibilities
  • Perform skilled nursing visits in patient homes
  • Complete Start of Care (SOC), Recertification, Resumption of Care (ROC), and discharge visits
  • Conduct patient assessments and develop individualized care plans
  • Perform medication management and patient education
  • Monitor vital signs and changes in condition
  • Coordinate care with physicians, therapists, case managers, and family members
  • Complete timely and accurate EMR documentation
  • Educate patients and caregivers on disease management and safety
  • Follow Medicare and home health regulatory guidelines
Qualifications
  • Active California RN license
  • Minimum 1 year RN experience preferred
  • Home Health experience strongly preferred
  • OASIS documentation experience preferred
  • Knowledge of Medicare home health guidelines
  • Current BLS certification
  • Valid California driver's license and reliable transportation
  • Strong clinical assessment and communication skills
