Position Summary:
We are seeking a compassionate and reliable Home Health Registered Nurse (RN) to provide skilled nursing visits to patients in Orange County and surrounding areas. This role requires strong clinical judgment, efficient documentation, and the ability to work independently in the field.
Responsibilities:
  • Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, and routine skilled nursing visits.
  • Conduct comprehensive patient assessments and develop individualized care plans.
  • Complete accurate and timely documentation (within 48 hours compliance)
  • Perform medication reconciliation at each visit and report changes.
  • Provide patient and family education based on diagnosis and care needs.
  • Coordinate care with physicians, care coordinators, and interdisciplinary team.
  • Ensure electronic visit verification (EVV), including clock in/out and patient signature.
  • Maintain compliance with Joint Commission, CDPH, and agency standards.
Qualifications:
  • Active California RN License (required)
  • Home Health experience preferred (OASIS documentation knowledge is a plus)
  • Current CPR certification.
  • Valid Driver's License and reliable transportation.
  • Strong clinical, organizational, and communication skills.
  • Ability to work independently and manage time efficiently.
  • Familiarity with EMR systems (MyUnity experience is a plus)
Requirements for Field Eligibility:
  • Current Physical Exam and TB Test/Clearance.
  • Immunization Records (Flu, Hep B, TDAP, MMR and Varicella)
  • Compliance with all credentialing requirements.
  • Ability to meet documentation and submission deadlines.
