ABOUT THE ROLE
Family Pillars Home Care is expanding our clinical team and seeking compassionate, reliable Registered Nurses to complete Start of Care (SOC), Resumption of Care (ROC), Re-certifications, and Skilled Nursing Visits in the community.

RESPONSIBILITIES
  • Complete Start of Care (SOC) assessments in accordance with state and agency guidelines
  • Perform Skilled Nursing Visits including wound care, medication management, patient education, and chronic disease monitoring
  • Develop and update individualized care plans
  • Communicate with physicians, case managers, and the interdisciplinary team
  • Document accurately and timely in the EMR
  • Provide high-quality, compassionate care to patients in their homes

REQUIREMENTS
  • Active Florida RN license
  • One year of nursing experience (home health preferred but not required)
  • Valid driver's license and reliable transportation
  • CPR certification
  • Level 2 background screening (or willingness to obtain)
  • Strong clinical assessment skills
  • Ability to work independently
