Matrix Home Care is seeking an experienced Registered Nurse (RN) to provide skilled nursing visits, including Start of Care (SOC), routine visits, follow-up visits, patient assessments, and care coordination throughout North Palm Beach, Palm Beach Gardens, Jupiter, and surrounding North Palm Beach County areas.
This is a field-based home care position ideal for an RN who is comfortable working independently in patients' homes, providing compassionate skilled nursing care, and completing accurate and timely clinical documentation.
Key Responsibilities
Provide skilled nursing care to patients in their homes.
Complete Start of Care (SOC), routine visits, follow-up visits, and nursing assessments as assigned.
Monitor patient condition and report any changes, concerns, or updates to the clinical team.
Provide patient and caregiver education based on the plan of care.
Follow physician orders, the established plan of care, and agency policies.
Communicate timely updates regarding patient status, care needs, and visit outcomes.
Complete all required clinical documentation accurately and timely.
Maintain professionalism, reliability, and compassionate patient care while working independently in the field.
Requirements
Active Florida Registered Nurse (RN) license required.
Minimum of 1 year of nursing experience preferred.
Home health, home care, skilled nursing, or field-based nursing experience strongly preferred.
Experience with Start of Care, routine visits, assessments, and clinical documentation preferred.
OASIS experience preferred, but not required.
Must be comfortable working independently in patients' homes.
Strong clinical judgment, communication, and documentation skills required.
Must be dependable, professional, and patient-focused.
