Location: Louisville, KY (Coverage: ZIP Codes 40214, 40215, 40216, 40258, 40272, 40118, with possible coverage throughout Metro Louisville)
Job Type: Travel Contract
Guaranteed Hours: 36 Hours/Week
Productivity: 30 Points/Week
Schedule: Monday–Wednesday | 8:00 AM–4:30 PM
On-Call: One weekend per month for Start of Care (SOC) and on-call visits
Mileage Reimbursement: Yes, between patient visits

Position Summary

We are seeking an experienced Travel Registered Nurse (RN) to provide skilled home health and hospice nursing care. The RN will perform OASIS assessments, deliver patient-centered care, and manage a home-based caseload while ensuring timely documentation and regulatory compliance.

Key Responsibilities
  • Perform Start of Care (SOC), Recertification, Resumption of Care (ROC), and Discharge OASIS visits.
  • Provide skilled nursing care to home health and hospice patients with diverse clinical needs.
  • Complete accurate and timely documentation in Homecare Homebase (HCHB).
  • Develop and update individualized care plans.
  • Collaborate with physicians and the interdisciplinary care team.
  • Participate in weekend on-call coverage as scheduled.
  • Meet productivity and quality standards while ensuring excellent patient outcomes.
Required Qualifications
  • Active Kentucky RN License.
  • Minimum 1 year of Home Health experience.
  • OASIS experience required.
  • Homecare Homebase (HCHB) experience required.
  • Strong clinical assessment and documentation skills.
