Job description

Position Overview
Inabia is seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join its Home Health team in Elkhart, Indiana. This role provides skilled nursing care to patients in their homes, supporting recovery, independence, and overall well-being.
The ideal candidate is organized, clinically competent, and comfortable managing visits and documentation in a home health environment.Key Responsibilities
  • Perform in-home nursing visits across Elkhart and LaGrange Counties
  • Complete Start of Care (SOC), Resumption of Care (ROC), Recertifications, and routine visits
  • Administer medications and treatments per physician orders
  • Assess patient conditions and report changes to the supervising RN or provider
  • Educate patients and caregivers on care plans, medications, and safety
  • Maintain accurate and timely clinical documentation
  • Coordinate care with interdisciplinary team members
  • Ensure compliance with agency policies, state regulations, and clinical standards
Systems & Documentation
  • Required experience with OASIS
  • Required experience with HCHB (Homecare Homebase)
Orientation Requirement
  • Must attend an in-person orientation session with the Talent Team at the Elkhart branch
  • Orientation is held on Mondays or Wednesdays
Qualifications
  • Active Indiana LPN license (required)
  • Home health experience preferred
  • Proficiency with OASIS and HCHB required
  • Valid driver's license and reliable transportation
