III. KNOWLEDGE AND RESPONSIBILITIES: The Case Manager is responsible for assisting the Foster Care Director/Placement Supervisor in the oversight of the Child Placements in Foster Homes including file maintenance.

The Foster Care Case Manager will:

A. Oversee and monitor the placements of children placed in Smith Agency Foster Homes.

B. Complete home assessments of the physical environment of homes for compliance with regulations

C. Act as a liaison with courts, county departments, schools, foster parents and all peripheral parties

D. Conduct home studies to assess ability of provider(s),

E. Provide and coordinate Case Management and protection for clients and families (where applicable)

F. Assist in intake process of new clients into agency homes

G. Complete, organize and maintain all assigned children files

H. Attend necessary meetings, staffing, home visits, and trainings to assure high quality of care for all individuals

I. Arrange and facilitate all required staffings and write required reports for individuals in the care of the Smith Agency foster homes

J. Attend management meetings and provider meetings

IV. SCHEDULE AND COMPENSATION:

The Case Manager's schedule is Monday through Friday, 9am to 5pm but must be available to be on call, which could include some work in the evenings and on weekends.

Requirements:

The qualified Case Manager shall hold a bachelorโ€™s degree in the social or behavioral sciences and if not as qualified as the placement supervisor, must be supervised by a qualified placement supervisor. A Case Manager with a bachelorโ€™s degree in a non-related field, he/she shall have at least two yearsโ€™ experience supervised by an individual with a Master of Social Work or other masterโ€™s degree in the social or behavioral sciences.
