Job description

Job Overview
Fairview is seeking a Family Medicine physician to join our primary care practice at M Health Fairview Clinic - Princeton. Join our thriving multi-specialty practice with onsite access to a robust group of specialty providers and services.
Onsite specialties: Family Medicine, Internal Medicine, Ob/Gyn, Orthopedics, Pediatrics, Podiatry, Sleep Medicine, Sports Medicine, General Surgery, Cardiology, ENT, Gastroenterology, Nephrology, Neurology, Oncology, Ophthalmology, Pulmonology, Radiology, Rheumatology, Urology, and more.
Onsite services: Rehab services, counseling, audiology, diabetic education, pharmacy, medication therapy management, lab, x-ray, mammography, ultrasound, infusion therapy, and bone densitometry.
Practice Details & Responsibilities:
  • 17 provider primary care practice, including 6 FM with OB physicians.
  • Patient encounters average 16-20 patients per day.
  • 34 patient contact hours per week.
  • Call Frequency: 5-6 times a month
  • Openings: 0.8 to 1.0 FTE - Flexible schedule
  • As part of Fairview's Accountable Care Organization (ACO), providers practice in a collaborative, team-based model focused on quality outcomes, care coordination, and population health management.
  • Examines patients, formulates differential diagnostic plans, defines and orders required diagnostic testing. Provides preventative, chronic disease, and acute care depending on the need of the clinic site. Ensures proper injury care and disease prevention, diagnosis, treatment, and recovery.
  • Prescribes and administers medications, performs routine vaccinations, and provides advice regarding personal health and hygiene.
  • Interprets examination findings and test results and implements treatment plans. Refers patient to medical specialist or another practitioner when necessary.
  • Completes medical record and electronic order management (via in basket); and completes cross coverage depending on site needs. Follows organization policies and procedures and protocols.
  • Actively participates in multidisciplinary team approach to case management
  • Participates in utilization and quality tools and activities that promote continuous improvement in delivery of patient care, patient outcomes or workflows.
  • Maintains knowledge of, and complies with, all relevant laws, regulations, policies, procedures, and standards.
  • Actively participates in creating and implementing improvements.
Required Qualifications
  • Graduate of an accredited ACGME/AOA Family Medicine residency training program
  • BC/BE with the ABMS or AOA
  • Licensed or ability to obtain licensure through the Minnesota Board of Medical Practice
  • Ability to obtain and/or maintain DEA certification for the State of Minnesota
  • Meet threshold criteria/qualification for Credentialing and Privileges
  • Board Certified in Family Medicine, or dual Board Certification in Internal Medicine/Pediatrics Minnesota license to practice as an MD or DO

M Health Fairview Clinic - Princeton is located approximately 60 minutes from Minneapolis and 30 minutes from St Cloud, MN. We are situated on the same campus as M Health Fairview Northland Medical Center.

The actual rate of pay offered within this range may depend on several factors, such as FTE, skills,knowledge, relevant education, experience, and market conditions. Additionally, our organization values pay equity and considers the internal equity of our team when making any offer. Positions withthe same min/max are flat rate jobs based on 40 hours/week. Compensation plans based on production can allow for higher pay than the range posted.
