About the Role
Sierra Health Care has been serving Las Cruces families since 1982 - and for over four decades, our nurses have been the steady presence patients and families count on most. As a Hospice RN in this hybrid role, you'll care for patients across home health, hospice, and palliative care, bringing skilled, compassionate nursing to people at some of the most significant moments of their lives. Key Responsibilities
  • Conduct assessments and develop individualized plans of care across home health, hospice, and palliative patients
  • Provide skilled nursing services including pain and symptom management
  • Educate patients and families on the hospice philosophy, end-of-life care, and comfort-focused approaches
  • Coordinate care with the interdisciplinary team and participate in IDG meetings and care conferences
  • Respond promptly to urgent patient and caregiver needs, including crisis intervention
  • Supervise and support LPNs and home health aides, including required supervisory visits
  • Complete accurate and timely clinical documentation in compliance with hospice Conditions of Participation
  • Provide on-call coverage as assigned

Requirements
What We're Looking For
  • Current, unencumbered RN license in New Mexico
  • Graduate of an accredited school of professional nursing
  • Two or more years of nursing experience preferred; hospice or palliative care experience a plus
  • Sound clinical judgment and the ability to work independently in the field
  • Compassionate, steady presence with sensitivity to cultural, spiritual, and emotional needs
  • Strong communication skills across patients, families, and care teams
  • Basic computer proficiency and willingness to learn hospice EMR documentation
  • Reliable transportation, valid driver's license, and auto insurance for home visits
