Job description

We are seeking a Bi-lingual Mental Health Case Manager to join our team! You will provide recovery-oriented rehabilitation services in an individual or family setting. The ideal candidate will possess a strong background in social work and behavioral health, demonstrating the ability to support individuals through various challenges. Our new case manager will be responsible for assessing client needs, developing personalized care plans, and providing ongoing support to ensure successful outcomes. This role requires excellent crisis management skills and the ability to mentor clients effectively.

Responsibilities:

  • Conduct comprehensive assessments to identify client needs and strengths.
  • Develop, implement and monitor individualized care plans tailored to each client's goals.
  • Provide crisis intervention and management to clients in urgent situations.
  • Utilize motivational interviewing techniques to empower clients toward a positive change.
  • Facilitate access to community resources, including Section 8 housing assistance and workforce development programs.
  • Offer support in addiction counseling and behavioral health services as needed.
  • Supervise and mentor clients in their personal development and recovery journeys.
  • Maintain accurate documentation of client progress and case notes in compliance with organizational policies.
  • Collaborate with multidisciplinary teams to ensure holistic care for clients.
  • Implement and review treatment plans.
  • Collaborate with additional treatment providers and placement agencies.
  • Ensure all documentation is completed in a timely manner.

Qualifications:

  • Bachelor's degree in social work, psychology, human resources or a related field. Master's degree preferred.
  • Proven experience in case management, mental health counseling or other related fields.
  • Familiarity with community mental health resources and motivational interviewing principles is highly desirable.
  • Excellent communication, interpersonal, verbal and organizational skills.
  • Must have current ANSA/CANS.
  • Experience with Section 8 housing is a plus.
  • Ability to work independently as well as part of a team-oriented environment.
  • Commitments to maintaining confidentiality and ethical standards in all client interactions.
  • Ability to navigate and document in an electronic health record.
  • Ability to build a rapport with clients.
  • Strong skills in crisis management and intervention techniques.
