Cardiology Consultants of Philadelphia (CCP) is a professional medical organization committed to the healing mission of Medicine and is part of a national cardiology platform, Cardiovascular Logistics (CVL). Together, we share the same mission to provide our patients with the highest quality cardiovascular care available. CCP specializes in providing comprehensive cardiology care and currently has over 30 locations throughout the Philadelphia region. We are currently hiring for a full-time Advanced Practice Provider to work at our Philadelphia, PA office. This position will be hybrid.
About the Role:
We are seeking a highly skilled Advanced Practice Provider (APP) to support the implementation of a new corporate-driven patient care protocol designed to improve outcomes for high-risk patients following cardiac events. This hybrid role blends clinical expertise, telehealth-based follow-up, and operational strategy.
The APP will conduct virtual telehealth visits, working closely with cardiology partners, research team, and internal corporate leaders to ensure high-quality follow-up and adherence to newly established care pathways.
Key Responsibilities:
  • Lead the clinical execution of new post-cardiac event care protocols for high-risk patients across CCP.
  • Perform telehealth visits to monitor symptoms, medication adherence, and recovery progress.
  • Monitor early warning signs and escalate patient concerns appropriately to reduce hospital readmissions.
  • Provide evidence-based patient education focused on cardiac recovery, lifestyle modification, and long-term risk reduction.
  • Document virtual encounters clearly and accurately in accordance with company and regulatory standards.
  • Collaborate with cross-functional teams to optimize workflow, identify implementation barriers, and support continuous program improvement.
  • Stay current on best practices, guidelines, and emerging evidence in cardiac care and telehealth delivery.

Requirements:
  • Must have a minimum of 3 years experience in a Cardiology
  • Lipid/blood pressure management experience preferred
  • Must have the ability to multitask
  • Computer knowledge is a must
  • Experience with an EMR system
  • Must have strong verbal and written communication skills
