Job description

Schedule: Full-time, 40 hours per week | Monday - Friday | 8am - 4:30pm
Department:  Thome PACE | Jackson, MI

What You'll Do:
The Homecare Scheduler will be responsible for scheduling of homecare services for PACE participants. 
  • Maintains homecare, holiday, on-call and weekend schedule for CENA/PCA staff.
  • Maintains homecare scheduling software PACECare 365 which includes but not limited to daily/weekly. scheduling, revamping schedule, and communication with family and participants. 
  • Communicates with participant care manager regarding CENA/PCA pick-up and trades.
  • Maintains service notes within the EMAR related to services provided to participants.
  • Works with contracted agencies to secure in-home staffing as needed.
  • Works with aide staffing with communication of care needs, scheduling questions or concerns.
  • May assist in participant homes when staff is needed.
  • Demonstrates active participation in the QI process such as chart reviews.

  • Prior Scheduling experience for a home based healthcare model.
  • Must be able to read, write, speak and understand the English language and follow written and oral instructions.
  • Must be CPR certified or willing to obtain.
  • At least one (1) year of experience working with the elderly.
