Medical Respite Coordinator : CHANCE

Community Helping All Negotiate Change Effectively (CHANCE) is a peer recovery support center that uses non-clinical, peer-based activities that support individuals to make the life changes necessary to recover from mental health and substance use disorder conditions.

Position objective:

The Medical Respite Care Coordinator will provide compassionate, peer-informed support and case management to shelter residents recovering from medical conditions. This role ensures smooth coordination between CHANCE, Samaritan Health Services, and other community partners to facilitate healing and transition.

The Medical Respite Care Coordinator works closely with Samaritan Health Services staff, the Shelter Director, and the CHANCE Leadership Team to manage the placement and care of residents in the Medical Respite Program. This individual performs intakes and exits, provides peer support, and helps residents meet medical, physical, and life goals.

Duties and responsibilities:

Coordinate with Care Hub.

Serve as the liaison between shelter staff, healthcare providers, and social service agencies.

Advocate for clients' needs within the healthcare system.

Assist with appointment scheduling and Rideline transportation.

Provide peer support and help residents set and achieve personal goals.

Support residents in transitioning from respite to shelter or permanent housing.

Assist with laundry and daily cleaning as needed to maintain a safe and hygienic environment.

Conduct intakes and exits for the Medical Respite Program.

Track and manage data in AWARDS/HMIS systems.

Maintain accurate documentation of resident progress and program outcomes.

Regularly attend and participate in team meetings and ongoing training  


Requirements

High school diploma or G.E.D. equivalent

Basic computer skills (Microsoft suite)

Must be able to pass a state background check

Excellent time management skills and ability to multitask 

Must follow C.H.A.N.C.E.'s Code of Conduct, and policies and procedures

Must maintain strict confidentiality

Skills:

Consistently maintain a professional demeanor and encourage individual respect

A team player who works collaboratively with others

Ability to work effectively with peers, community partners, coworkers, general public, medical personnel, corrections personnel, police, and groups without bias

Comfortable with and open to learning and applying new skills

Knowledge of community and social service agencies preferred

Sufficient manual dexterity and physical ability to perform assigned tasks, or able to use adaptive equipment to perform assigned tasks, if needed

Ability to work in an environment where people may be hostile, while contributing to a therapeutic environment

Knowledge of de-escalation methods or desire to be trained in them; feel comfortable asking for help if not trained in de-escalation 

Ability to maintain accurate records and necessary documentation
