Lifeworks is hiring a full-time Assistant Program Manager to work with Individuals with ID/DD in one of our Residential Programs. We are a nonprofit human service agency that provides adults with developmental disabilities with the opportunity to live engaged and meaningful lives. We're searching for an enthusiastic individual who will bring their personality, an independent work ethic, and a strong belief in enhancing the lives of those we serve. *Candidates must have related or direct experience working with the community we serve.
Essential Duties:
  • Responsible for assisting with all hands-on direct care which includes but not limited to: showering, toileting, shaving, brushing teeth, meal-time assistance
  • Ensures health and safety of the Individual that including Personal care.
  • Provide education training, counseling, and physical assistance when necessary to achieve ISP goals in the following areas: functional academics, domestic skills health and personal hygiene, social and leisure skills, communication skills, community mobility, self-esteem, money management.
  • Communicate with family, friends, guardians, advocates, volunteers, employers to facilitate consistency of programming and support.
  • Develop, write, and present annual ISPs and semiannual agreements in consultation with other residential staff, Program Manager, and Director.
  • Perform case management duties.
  • Provides transportation to individuals as necessary.
  • Scheduling/supervision of staff.
Requirements:
  • Experience working with Individuals with ID/DD
  • Bachelor's degree from four-year college or university; related experience preferred
  • Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks
  • Valid Driver's License required
  • Valid MAP certification is preferred
  • Excellent verbal and written communication skills
