This position is fully remote, allowing you to work from the comfort of your own home while maintaining a flexible schedule. As part of a hospice center, you will collaborate with a multidisciplinary team consisting of psychiatrists, psychologists, nurses, and case managers dedicated to the holistic care of our clients. This collaborative setting enhances communication and continuity of care, ensuring optimal outcomes for those we serve.

• Perform comprehensive mental health assessments, including psychosocial evaluations, diagnostic interviews, and risk assessments to better understand each client's unique experiences and needs.
• Engage with clients to develop personalized treatment plans that incorporate their specific goals, health conditions, and cultural backgrounds.
• Utilize evidence-based therapeutic interventions such as CBT, DBT, and solution-focused brief therapy (SFBT) in various settings including individual, couple, family, and group therapy sessions.
• Provide crisis intervention services and work closely with healthcare professionals to ensure coordinated and prompt care for clients in acute distress.
• Maintain thorough and updated clinical documentation, including progress notes, treatment plans, and discharge summaries in compliance with clinic policies and regulatory standards.
• Deliver psychoeducation to clients and their families to empower them with the skills and knowledge needed to navigate their mental health journey effectively.
• Contribute to outcome assessments that track treatment effectiveness and adjust care strategies as necessary to promote recovery.
• Stay current on mental health research, best practices, and emerging trends, actively incorporating new insights into your clinical work.
• Adhere strictly to ethical guidelines to maintain confidentiality and privacy of client information.

You will primarily work with clients who have experienced various forms of trauma and adversity. Focused on providing culturally sensitive care, you will interact with clients from diverse backgrounds, ensuring that all individuals feel respected and understood throughout their therapeutic journey. Your role is pivotal in assisting clients in overcoming their challenges and achieving their personal goals for mental health and wellness.

• A Master’s degree in Social Work is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Texas.
• Demonstrated ability to work both independently and collaboratively within a multidisciplinary healthcare team.
• Strong interpersonal and communication skills, with an aptitude for building rapport with clients from various cultural backgrounds.

• Experience in trauma-informed care practices is highly preferred.
• Prior work experience in hospice or palliative care settings is a plus.
• Proficiency in utilizing telehealth technology for remote therapy sessions.

This position offers an annual salary in the range of $70,000 to $84,000, commensurate with experience and qualifications.
