Work Environment

Key Responsibilities
  • Conduct thorough psychosocial assessments through interviews, observations, and comprehensive reviews of medical records, evaluating emotional, social, and environmental factors affecting patient recovery.
  • Provide one-on-one counseling and therapeutic support to patients and their families, addressing vital adjustment issues and fostering emotional resilience during this challenging time.
  • Collaborate closely with a multidisciplinary rehabilitation team to craft individualized treatment plans that holistically incorporate patients' psychosocial needs and therapeutic goals.
  • Assist patients and families in accessing relevant community resources such as support groups, vocational rehabilitation programs, and housing assistance, ensuring comprehensive care beyond the hospice environment.
  • Deliver pertinent information and education to families regarding available community resources and post-rehabilitation services to facilitate smoother transitions to home or other care settings.
  • Actively participate in discharge planning meetings, ensuring the smooth transition of care and supporting patients and families as they move towards recovery and re-establishing everyday life.
Patient Population / Client Focus

This role primarily serves patients in hospice care, including those facing significant end-of-life challenges. You will provide emotional and practical support to a diverse patient population, often involving individuals with chronic illnesses, terminal diagnoses, and varying degrees of psychosocial distress. Qualifications
  • A Master's degree in Social Work from an accredited program is required.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW) is essential to practice in this role.
