Your primary responsibilities will include:

• Conducting comprehensive clinical assessments and developing individualized treatment plans based on best practices for hospice care.
• Administering various therapeutic interventions, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), specifically catered to the emotional challenges faced by terminally ill patients and their families.
• Maintaining accurate clinical documentation in compliance with HIPAA regulations, ensuring confidentiality and security of patient information.
• Collaborating with an interdisciplinary team of health professionals, including physicians, nurses, and spiritual counselors, to provide coordinated and effective caregiver support.
• Mentoring and supporting less experienced counselors in understanding the nuances of providing hospice care.
• Facilitating support groups and workshops for families, focusing on coping mechanisms and resources available during the end-of-life journey.
• Staying abreast of current research and methodologies in mental health and hospice care, applying these insights to enhance patient outcomes.

In this role, you will work closely with a diverse population that includes individuals facing terminal illnesses and their families. Your work will be crucial in alleviating emotional pain, fostering resilience, and helping families navigate complex emotional challenges during difficult times. You will play an integral role in ensuring that patients feel supported, understood, and valued as they approach the end-of-life stage.

The ideal candidate will possess:

• A Master's degree in Social Work (MSW) from an accredited institution.
• A valid LCSW license in the state of Illinois.
• Demonstrable experience, with a minimum of three years in a clinical setting focusing on providing psychotherapy.
• Comprehensive knowledge of solution-based psychotherapy techniques.
• Strong verbal and written communication skills.
• The ability to collaborate effectively in a remote team environment.

While the minimum qualifications will get your foot in the door, the following experiences are preferred:

• Previous experience working in a hospice or palliative care setting.
• Familiarity with the unique psychological aspects of end-of-life care.
• A background in providing bereavement support to families.
