• Conduct comprehensive clinical assessments to determine the psychosocial needs of patients and families within our hospice program.
• Create individualized treatment plans that address the unique challenges faced by patients in hospice care, incorporating evidence-based practices.
• Implement solution-focused therapeutic interventions that help patients process their experiences and emotions.
• Maintain accurate and timely clinical documentation in accordance with organizational policies and state regulations.
• Collaborate actively with a multidisciplinary treatment team, including physicians, nurses, and other healthcare professionals to ensure holistic care delivery.
• Provide ongoing education and support to both patients and their families, ensuring they have the resources and understanding needed through the hospice experience.
• Stay informed on the latest mental health research and treatment methodologies to continually elevate the standard of care provided.

In this role, you will work primarily with patients in a hospice setting, supporting individuals facing terminal illnesses. This includes addressing a range of emotional, social, and spiritual concerns. You will help patients navigate their end-of-life journey, making a meaningful impact as they deal with complex life transitions, grief, and loss. Your expertise in solution-focused techniques will enable you to foster resilience and facilitate discussions that can help enhance patients’ overall well-being during this critical time.

• A Master's degree in Social Work from an accredited institution.
• A valid Licensed Clinical Social Worker (LCSW) license in the state of Florida.
• Demonstrated knowledge of solution-based psychotherapy interventions relevant to hospice care.
• Exceptional communication skills, both written and verbal, with an aptitude for active listening and compassionate engagement.

• Experience working in a hospice or palliative care setting, with a strong understanding of the emotional and psychological needs of patients during terminal phases of life.
• Familiarity with electronic health records (EHR) systems to streamline clinical documentation and communication.
• Previous involvement in a multidisciplinary treatment environment.

This position offers a competitive salary ranging from USD 80,000 to 100,000 annually, commensurate with experience.
