• Conduct thorough assessments of patients' psychological and emotional needs to develop personalized care plans.
• Implement evidence-based therapeutic interventions, including grief counseling, family therapy, and crisis intervention.
• Maintain detailed and confidential records of client progress and treatment plans, ensuring compliance with all regulatory standards.
• Collaborate with healthcare professionals—including physicians, nurses, and spiritual care advisors—to create a cohesive care experience for patients and families.
• Stay abreast of the latest research, trends, and best practices in social work and hospice care to enhance service delivery.

In this role, you will work with a diverse client population, primarily focusing on patients experiencing life-limiting illnesses and their families. Our clients often deal with complex emotions related to grief, loss, and anxiety, requiring an empathetic and skilled approach to help alleviate their fears and support their emotional well-being. You will play a crucial role in guiding these individuals through their journey, providing necessary resources and emotional backing as they navigate their circumstances.

• A Master’s degree in Social Work from an accredited institution is required.
• An active LCSW license in the state of Tennessee is essential for this role.
• Exceptional verbal and written communication skills; ability to engage clients in meaningful dialogue is critical.
• Experience in hospice, palliative care, or related fields is highly valued.

While the essential qualifications focus on your educational background and licensure, candidates with previous experience in trauma-informed care, crisis intervention, and family therapy will thrive in this position. Familiarity with electronic health record (EHR) systems and telehealth practice will enhance your effectiveness as you provide care to patients remotely.
