• Conduct detailed clinical assessments of patients to identify their psychosocial needs, risks, and strengths, establishing individualized treatment plans aimed at enhancing their quality of life.
• Utilize evidence-based therapeutic modalities, including cognitive-behavioral therapy (CBT) and solution-focused brief therapy (SFBT), to address mental health symptoms such as depression, anxiety, and grief that often accompany terminal illness.
• Maintain patient confidentiality and comply with HIPAA regulations by managing accurate and timely documentation of clinical interactions and treatment plans.
• Engage in multidisciplinary team meetings with physicians, nurses, and other healthcare providers to ensure comprehensive care and discuss patient progress.
• Participate in regular case consultations and peer review sessions to continually improve and adapt treatment approaches as needed.
• Provide education and support to patients and their families regarding available resources, coping strategies, and the dynamics of terminal illness, empowering them to make informed decisions about care.
• Stay current with the latest research in mental health, adapting best practices into your work to improve patient outcomes.

Your primary focus will be on patients receiving hospice care, individuals who are confronted with terminal diagnoses requiring compassionate and specialized support. • A Master's degree in Social Work from an accredited institution is required to confirm your expertise in the field.
• Active license as a Licensed Clinical Social Worker (LCSW) in the state of Michigan, demonstrating your professional qualifications and readiness to practice.
• Solid knowledge of various therapeutic modalities, particularly those effective in managing grief, PTSD, and complex emotional responses to terminal illness.

The salary range for this position is set between USD 90,000 and 104,000 annually, commensurate with experience and qualifications.
