Position Summary
 The Supervisor Central Services Clinical RN provides clinical and operational oversight for the centrally managed nursing team supporting system-wide functions including clinical triage, inbox management,
and transitions of care coordination. This role ensures high-quality, patient-centered remote clinical services through effective team leadership, adherence to evidence-based protocols, and collaboration with care teams across the organization. The supervisor is responsible for staff performance, workflow
optimization, and ensuring consistent delivery of safe, efficient, and coordinated care that supports improved patient outcomes and operational priorities.
Job Description

PRIMARY DUTIES AND RESPONSIBILITIES

1. Leads and supervises the Central RN team to ensure consistent delivery of high-quality clinical triage, patient communication, and care coordination services across the organization.

2. Provides clinical oversight and guidance to staff performing telephone triage and remote clinical care, ensuring adherence to established protocols, evidence-based practices, and regulatory requirements.

3. Monitors team workflows, productivity, and service levels to ensure timely response to patient inquiries, test results, medication refills, and other clinical communications.

4. Partners with clinical and operational leaders to optimize centralized nursing workflows supporting triage, inbox management, pre-visit planning, and transitional care outreach.

5. Oversees the accuracy and quality of documentation within the electronic medical record (EMR), ensuring compliance with best practices, regulatory standards, and organizational workflows.

6. Supports quality improvement initiatives by monitoring clinical quality measures, identifying opportunities for improvement, and guiding the team in closing care gaps.

7. Provides coaching, mentoring, and performance management to nursing staff to support professional development, clinical competency, and engagement.

8. Collaborates with providers and interdisciplinary care teams to promote seamless care coordination and ensure patients receive timely and appropriate care interventions.

9. Facilitates post-discharge outreach processes and care transition activities to support reduced readmissions and continuity of care.

10. Promotes effective utilization of healthcare resources and patient/member benefits through appropriate triage, care coordination, and patient education.

Education

  • Bachelor's degree in Nursing
  • Current, unrestricted Registered Nurse (RN) license
  • BLS certification required

Experience

  • 5+ years of clinical nursing experience, preferably in a primary care or ambulatory care setting
  • Prior supervisory or leadership experience
  • Telephone triage or remote care experience
  • Experience in care coordination or population health programs
  • Knowledge of healthcare operations, particularly within primary care or ambulatory care environments
  • Knowledge of clinical triage protocols and care coordination processes
  • Understanding of quality measures and strategies for improving patient outcomes through quality gap closure
  • Understanding of clinical quality measures (HEDIS) Knowledge of coding concepts including CPT, ICD10, and HCC
  • Familiarity with EHR documentation standards and quality reporting processes
