Job Summary

The Behavioral Health Home Registered Nurse Care Manager is responsible for coordinating integrated physical and behavioral healthcare services for adults and children enrolled in MaineCare Behavioral Health Home Services. Working as part of an interdisciplinary team, this role provides nursing assessments, care coordination, health education, clinical consultation, and support to help members achieve improved health outcomes and overall well-being.

Qualifications

Associate or Bachelor of Science in Nursing (BSN) required; advanced degree preferred
Active, unrestricted Registered Nurse (RN) license meeting Maine requirements
Current CPR/BLS certification required

Experience in care management, case management, integrated care, population health, or community-based healthcare settings

Collaborate with members, families, providers, and community partners to develop and implement individualized Plans of Care
Provide health education, medication support, chronic disease management guidance, and self-management coaching
Coordinate referrals, follow-up care, hospital transitions, and access to healthcare and community resources
Monitor health outcomes, preventive screenings, care gaps, and required health measures while supporting quality improvement efforts
Participate in interdisciplinary team meetings, clinical consultations, and care planning activities to promote coordinated, whole-person care
Maintain accurate and timely documentation while ensuring compliance with MaineCare, HIPAA, nursing standards, and agency policies
Support continuity of care through ongoing member engagement, advocacy, and coordination across healthcare settings
