Located in Reading, Pennsylvania, Madina Care Consulting operates in a remote work setting, allowing for flexibility while maintaining a strong connection with our hospice center team. This role involves close collaboration with healthcare professionals, including physicians, nurses, and support staff, all dedicated to ensuring comprehensive care for our patients. • Conduct comprehensive assessments of patients’ mental health, evaluating emotional, behavioral, and social functioning to identify areas of need.
• Develop individualized treatment plans that are evidence-based and culturally sensitive, aligning with the unique circumstances of each patient.
• Provide ongoing psychotherapy and therapeutic interventions, catering to a range of mental health conditions such as depression, anxiety, and PTSD.
• Collaborate with a multidisciplinary team, including physicians and nursing staff, to coordinate and enhance patient care.
• Regularly monitor and document patient progress, adapting treatment approaches based on patient feedback and clinical outcomes.
• Educate patients and their families about mental health conditions, treatment options, and community resources available to support their journey.
• Uphold ethical standards and ensure confidentiality in all patient interactions, fostering a trusting relationship with clients.

This position primarily focuses on patients receiving hospice care, who often face complex emotional challenges. The LCSW will work with individuals and their families as they navigate the psychological impact of serious illness and the end-of-life process. Your expertise will be instrumental in providing support and facilitating conversations around coping strategies, grief, and adjustment to illness.

• A Master's degree in Social Work from an accredited institution is required.
• Current state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of direct experience in providing psychotherapy and mental health services.
• Demonstrated ability to effectively communicate and build relationships with diverse patient populations and healthcare professionals.
• Proficiency in conducting assessments, developing evidence-based treatment plans, and leveraging electronic health records (EHRs) for care coordination.
• In-depth knowledge of various mental health conditions and an understanding of the complexities faced by hospice patients and their families.
• Commitment to maintaining confidentiality and adhering to high ethical standards in social work practice.

• Experience in hospice or palliative care settings is highly desirable, as well as familiarity with the specific emotional and psychological needs of patients in these situations.
• Proficient use of healthcare software and EHR systems to manage patient records efficiently.
• Advanced skills in formulating treatment plans tailored to individual patient needs, based on evidence-based practices.
