Are you an experienced RN or LCSW with a deep understanding of patient needs and a passion for ensuring they receive the right care at the right time?

DaVita Integrated Kidney Care (IKC) is seeking a dedicated and knowledgeable RN or LCSW with strong clinical acumen to step into a pivotal role as a Palliative Care Clinician. This is an opportunity to play a vital part in specializing in and growing a national kidney care palliative program.

You will serve as a subject matter expert and the central point of contact between our palliative care team and broader kidney care teams. As a member of our team, you will be accountable for triaging complex kidney patients and ensuring a seamless transition to the appropriate palliative resources.

DaVita Integrated Kidney Care (DaVita IKC) is the integrated care division of DaVita Inc. working on DaVita's vision to provide integrated care to all ESRD patients, who are some of the most medically complex and vulnerable patient populations in the US. Our dedicated nurses, care coordinators, nurse practitioners (NPs) and business professionals integrate and manage care for patients with late-stage chronic kidney disease (CKD) and end stage renal disease (ESRD) across the US each month.

We've proven that integrating care achieves the triple aim of improved patient quality of life, better outcomes and lower total cost of care.
