• Conduct in-depth clinical assessments for patients, identifying their mental health needs and strengths.
• Develop and implement tailored treatment plans that align with the unique circumstances of patients in hospice care.
• Utilize evidence-based therapeutic interventions, including solution-focused and cognitive-behavioral therapies, to support patients and families.
• Maintain thorough and accurate clinical documentation, ensuring compliance with regulatory standards and organizational policies.
• Collaborate with a diverse treatment team, engaging in regular case reviews and multidisciplinary meetings to optimize patient care.
• Educate patients and their families on mental health resources and coping strategies, providing ongoing support as they navigate the end-of-life process.
• Stay informed on the latest research, methodologies, and developments in social work and hospice care to enhance your professional practice.

In this role, you will primarily work with patients receiving hospice care and their families. This population often experiences a range of mental health challenges, including depression, anxiety, and grief, which requires a nuanced and empathetic approach. Your guidance and therapeutic interventions will be instrumental in helping individuals cope with difficult emotions and navigate this critical stage of life while maintaining dignity and hope.

• A Master's degree in Social Work from an accredited institution is required.
• Possession of a valid LCSW license in the state of New York.
• Proficiency in solution-based psychotherapy and a strong understanding of best practices in hospice care.
• Exceptional communication and problem-solving skills, with an emphasis on compassionate client interaction.

• Previous experience in hospice, palliative care, or a similar setting is highly regarded.
• Familiarity with electronic health record (EHR) systems for clinical documentation and patient management.
• Adeptness in conducting telehealth sessions, as this role may involve virtual interactions with clients and families.
