This candidate must reside in New Hampshire.
This is a full time, remote position with the occasional need to travel to the New London office. The ideal candidate is self-motivated, tech-savvy and an excellent communicator.
We are looking for a team player who is willing to jump in and help, is flexible, able to prioritize and stay organized. A detail oriented Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our Intake Department for 40 hours each week. This person will assist with both Homecare and Hospice referrals.

Responsibilities for Intake Licensed Practical Nurse (LPN) or Registered Nurse (RN):
  • Accurately enter patient referral information into the electronic medical record system
  • Accurately enter all incoming patient care orders and notify appropriate team members as necessary
  • Navigate multiple electronic referral software portals
  • Communicate with discharge planners, physician offices and liaisons regarding patient referrals while maintaining positive relationships
  • Triage all incoming phone calls
  • Communicate effectively with patients and patient care givers

Requirements for Intake Licensed Practical Nurse (LPN) or Registered Nurse (RN):
  • Active NH Licensed Practical Nurse (LPN) or Active NH Registered Nurse (RN) License
  • Must reside in New Hampshire
  • 2 years of Health Care experience
  • Knowledge of medical terminology, familiarity with a medical office setting, and/or triage
  • Knowledge of Home Health/Hospice
  • Must be comfortable working in a remote environment
  • Proof of Flu vaccination
  • Reliable transportation
  • Valid driver's license
  • Automobile insurance
  • Physical exam within the last year
  • Two-step TB test
