Job Title: RN Case Manager
Location: 100% Remote
Duration: 12+ months
License Required: Active & unrestricted Michigan RN license


The RN Case Manager serves as the primary point of contact for members, caregivers, and providers, coordinating a multidisciplinary team to deliver holistic, person-centered care. Using telephonic and digital communication, the RN leads assessment, care planning, implementation, and ongoing evaluation to support improved health outcomes across the care continuum.


Key Responsibilities

·       Coordinate a multidisciplinary team (Social Workers, Dietitians, Pharmacists, Clinical Staff, Medical Directors) to meet member needs.

·       Use the case management process to assess, develop, implement, and evaluate individualized care plans.

·       Assess health, psychosocial needs, cultural preferences, and support systems.

·       Engage members/caregivers to identify barriers, close care gaps, and promote self-management.

·       Coordinate care among providers, agencies, and family supports.

·       Monitor progress toward goals and adjust care plans as needed.

·       Support safe transitions of care, including discharge preparation and follow-up education.

·       Arrange DME and transportation as needed and document all interactions accurately.

 

Education & Experience

·       Nursing Diploma or Associate Degree required; BSN preferred.

·       3 years of clinical nursing experience in acute/post-acute/community settings required.

·       1 year of case management experience in managed care preferred.

·       Experience providing telephonic or digital care preferred.

 

Licenses & Certifications

·       Active, unrestricted Michigan RN license required.

·       CCM required or to be obtained within 18 months of hire.

·       CCP certification preferred.

 

Skills & Competencies

·       Strong critical thinking, decision-making, and problem-solving abilities.

·       Excellent time management, organization, and ability to work across multiple systems.

·       Proficient in Microsoft Office and digital communication platforms.
