Mind and Body Connect Wellness Center is seeking a dedicated and compassionate licensed mental health professional to join our team as a full-time or part-time Mental Health Counselor. Located in Scottsdale, Arizona, Mind and Body Connect Wellness Center is a comprehensive community health clinic providing an array of essential medical services, including obstetrics, pediatrics, general medicine, family planning, and dentistry. Our experienced team is committed to addressing various social issues through education, intervention, and personalized management plans. We prioritize confidentiality and accept Medicaid, Medicare, and most private health insurance policies.

This position will primarily be remote, enabling flexibility while ensuring accessibility for our diverse patient population. As part of a Federally Qualified Health Center (FQHC), the Mental Health Counselor will contribute to a mission-driven organization focused on compassionate care.

• Collaborate with primary care practitioners to assess, manage, and coordinate patient mental health and psychological concerns; become an integral member of the primary care team.
• Conduct thorough patient intakes, emphasizing diagnostic and functional evaluations, and develop treatment plans aligned with the provider's recommendations.
• Deliver targeted mental health solutions, adapting interventions as necessary to match the evolving needs of patients.
• Provide guidance to primary care providers regarding patients requiring specialized mental health care versus those who can be addressed within primary care.
• Implement follow-up procedures to monitor patients' progress and well-being, making adjustments to treatment methods as required.
• Create and implement crisis prevention strategies to support patients' functionality during challenging periods.
• Identify at-risk individuals and work to develop intervention programs aimed at preventing mental health deterioration.
• Supervise the delivery of behavioral health services, creating linkages with external treatment providers when necessary, with patient consent.
• Maintain accurate records of patient progress and diagnostic information to ensure effective treatment outcomes.
• Keep primary care physicians informed of patients' evolving needs and collaborate to refine treatment plans that align with clinical best practices.
• Utilize behavior modification techniques to assist patients at risk of physical health issues in adopting healthier lifestyle choices.
• Provide educational resources and self-management skills to empower patients in their treatment and recovery journey.
• Consult with clinic management and team members to optimize outcome evaluations and program effectiveness.
• Support patient adherence to prescribed medical treatments and address any medication-related concerns.
• Uphold the ethical standards outlined in the Code of Ethics for Clinical Professionals, ensuring patient rights and dignity are respected.
• Perform other duties as assigned, contributing to the overall effectiveness of the clinic.

The Mental Health Counselor will work with a diverse patient community, including individuals facing emotional distress, mental health disorders, and complex social issues. Our focus on holistic care requires sensitivity to cultural nuances and an understanding of personal and social challenges faced by patients. The counselor will play a pivotal role in enhancing patient outcomes through tailored support and interventions.

• A minimum of a Master’s Degree in Clinical Counseling or a related field.
• Current and valid Licensed Professional Counselor (LPC) license.
• Strong knowledge of mental health assessment, treatment planning, and therapeutic modalities such as family therapy, group therapy, and psychotherapeutic techniques.
• Ability to conduct diagnostic evaluations and apply appropriate therapeutic interventions effectively.
• Demonstrated commitment to ethical practice and professional standards in mental health care.
• Excellent interpersonal and communication skills, fostering trust and collaboration.
• Ability to work both independently and as part of a multidisciplinary team dedicated to patient health.
• Thorough understanding of community resources and referral pathways to support patient needs.
• Fluency in both English and Spanish is highly preferred, enhancing our reach in this multicultural community.

While not a requirement, experience working in a community health setting, particularly with underserved populations, is highly desirable. Familiarity with Federally Qualified Health Centers and knowledge of Medicaid and Medicare processes would be an asset.

We offer a competitive salary range of USD 80,000 to 100,000 annually, commensurate with experience and qualifications.
