Nurse Practitioner Opportunity

$7,500 Sign-on bonus for external candidates.

Position Highlights
  • Improve the quality of health for Medicare Advantage members across the states served by helping to monitor and manage the members alongside the primary care teams.
  • Use audio and visual technology to complete patient examinations, evaluations, assessments, diagnoses, and treatment.
  • Documenting/Charting patient information in line with current policies and procedures.
  • Hours are Monday-Friday, 8am-5pm CST
Primary Responsibilities:
  • Conduct assessments including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam
  • Patients will be seen via telemedicine visits
  • Perform evidence-based practice screenings including point of care testing (as appropriate)
  • Identify diagnoses to be used in care management and active medical management in the furtherance of treatment
  • Perform transition of care visits for patients recently discharged from acute or post-acute settings
  • Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment
  • Communicate findings in your assessment that will be used to inform member's PCP of potential gaps in care
  • Identify urgent and emergent situations and intervene appropriately
  • Educate members on topics such as disease process, medication, and compliance

Required Qualifications:
  • Master's degree (MSN) and certification by one of the Nurse Practitioner certifying bodies (ANCC or AANP) as a Family Nurse Practitioner (FNP)
  • Active DEA license with no restrictions in 1 State
  • Must Reside in one of the following States - KS, MO, WI, IN, OH, VA, NC, TN, SC, GA
  • Minimum of three (3) years of in person clinical NP experience (post-graduation)
  • Able to apply for Autonomous Nursing Privileges based on State Regulations
  • Must have access to high-speed internet and phone lines.
Preferred Qualifications:
  • Previous Experience with Epic.
  • Multiple Licenses in the following States preferred (KS, MO, WI, IN, OH, VA, NC, TN, SC, GA) with ability to obtain.
  • Experience in managed care environment.
  • Experience working in a virtual setting.

Compensation for this specialty generally ranges from $109,500 - $164,000 annually. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience, and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401k contribution (all benefits are subject to eligibility requirements).
