This position operates within a community health clinic setting, providing essential services to a broad spectrum of patients. Although the role is remote, collaboration and communication are facilitated through advanced telehealth technologies and integrated health platforms. You will work closely with multidisciplinary teams, including physicians, nurses, and case managers, ensuring that every patient's psychosocial needs are systematically addressed in their care plans.

• Manage an assigned caseload of patients, conducting thorough evaluations to identify social, emotional, financial, and cultural factors affecting their treatment and recovery.
• Develop and implement comprehensive treatment plans to address complex psychosocial challenges, ensuring patients receive the support they need to transition effectively through the continuum of care.
• Lead interdisciplinary healthcare teams in addressing protective service issues, life care decisions, and advance directives, thereby promoting patient autonomy and informed decision-making.
• Provide crisis intervention to patients and their families, identifying immediate needs and mobilizing resources to mitigate distress and facilitate recovery.
• Collaborate with case managers to ensure that patients and families have access to essential educational resources, referrals, and follow-up services tailored to their specific circumstances.
• Supervise and mentor junior staff, including MSW interns and volunteers, ensuring growth and excellence in service delivery across the team.
• Participate actively in specialized team rounds and patient care conferences, contributing valuable insights and fostering cohesive care collaboration.
• Maintain compliance with all regulatory requirements and internal protocols, ensuring a seamless, efficient, and high-quality delivery of psychosocial services.
• Be available for on-call responsibilities as designated by the department director to address urgent matters and support team initiatives effectively.

This role focuses on a diverse population, including individuals and families facing complex psychosocial challenges. The patients range in age and background, necessitating a culturally sensitive approach that recognizes and respects each individual's unique circumstances and needs. You will be expected to foster trusting relationships with patients, empowering them to take an active role in their healthcare journeys.

• A valid and unrestricted LCSW license in the state of California.
• A minimum of a bachelor's degree in social work from an accredited institution; a master’s degree (MSW) is preferred.
• At least two years of experience in a clinical social work setting, particularly in a healthcare environment.
• Strong analytical skills to assess and manage intricate cases involving various psychosocial factors.
• Excellent interpersonal and communication skills to effectively interact with patients, families, and healthcare professionals.

Experience in a remote healthcare setting is highly desirable. Familiarity with electronic health record (EHR) systems and telehealth methodologies will enhance your ability to coordinate care effectively. Knowledge of community resources and services is also beneficial for supporting patients and families in accessing necessary programs.

This full-time position offers a competitive salary range of USD 120,000 to 130,000 annually, commensurate with experience and qualifications.
